Office visits for dental treatment are always by appointment only; in this way, proper attention can be given to the scheduled patient.

Our office staff is trained to handle your telephone inquiries. If you have an emergency, please give the receptionist as much information as possible regarding the nature of your trouble. Every effort will be made to render treatment as soon as possible.

If you are a new patient, please inform the receptionist that you are new to the office. An examination and cleaning appointment will then be scheduled.



Payments

There is a need to establish a definite understanding regarding payment for dental care received. We ask at the end of each office visit that your account be settled in full. Please realize that by paying at the time service is rendered you are helping us decrease the administrative costs of the billing and collection processes. The patient ultimately benefits from this decreased overhead.

If your treatment plan necessitates a substantial investment on your part, an individual payment program can be designed for you over an agreed upon period of time. If you are having financial difficulties, please let us know. If you can talk to us about the problem and how long you expect it to last, we may again be able to make a suitable financial arrangement. Be assured, that all patient information is held in the strictest of confidence.

To those of you with dental insurance; payment is also expected at the end of each office visit. As your dental treatment is completed the services provided will be submitted to your insurance company and the benefits will be directed to you. Remember, your insurance is a contract between you and your insurance carrier and as such you should receive the benefits from your own carrier directly.

Insurance

Dental insurance is a benefit which many of you may have. Dental insurance, as with any kind of insurance, is a contract between you and your insurance carrier. We will do our very best to make sure you receive all the benefits for which you are entitled within the constraints of your policy. Some dental therapies may be beyond the scope of your insurance coverage; for this and any other costs you, as the patient, will be responsible.

The patient has certain obligations with regard to his/her insurance; the first of which is to provide us with your insurance card for documentation. The second responsibility is to provide us with any secondary insurance information you may have through your spouse or partner. We are seeking to actively involve you with your claim. This office will fill out your claim form and submit it at no cost to you. Please realize that this service requires a considerable amount our time. Your cooperation in fulfilling your obligations is necessary and greatly appreciated.

Cancellations

Along with your treatment plan, a schedule of appointments will be reserved for you. The length and sequencing of the appointments is calculated so that your dental care is provided in the proper manner.

Broken or cancelled appointments may upset your treatment sequence such that a whole new sequence may have to be calculated. We realize there are times when one just cannot keep an appointment. If this occurs to you, we only ask that you let us know as soon as possible.

Therefore, if you must cancel an appointment, please give us at least 24 hours (or sooner) notice.

Your consideration will be appreciated not only by us but also by your fellow patients who desire treatment and may therefore use your time.

No smoking is allowed in this office.

X-rays are the property of the office. Copies of your radiographic images will be released to you or any other parties you may designate or to other dentists, physicians or insurance companies for consultation or documentation purposes only.

We have no objection to your seeking a second opinion. Please feel free to discuss this with us if you desire. We will arrange a consultation or forward the necessary records to the consultant of your choice.
